Client Agreement

Transitional Housing Programs

Admission Criteria

  1. Clients must be at least eighteen (18) years of age.
  2. Clients may not be a registered sex offender. No Exceptions.
  3. Clients must be physically and emotionally able, and willing to work. If a client is unable to work, that client must volunteer for 20 hours a week. 
  4. Clients must be willing to abide by Keaton’s Place rules and policies.
  5. Clients at Keaton’s Place must be able to accurately and appropriately self-administer all physical and mental health medications, as prescribed.
  6. Clients must provide a $100 deposit before moving in, unless other arrangements have been discussed and agreed upon by Keaton’s Place staff.

Discharge Criteria

  1. If a client has an episode of alcohol and/or other drug consumption, ingestion, intoxication or a failure of a Breathalyzer or drug test it is grounds for discharge from transitional housing. 
  2. Failure to comply with Keaton’s Place rules and policies is grounds for termination from transitional housing.
  3. The sharing of ANY prescribed medication between clients will result in immediate discharge of transitional housing for all involved parties.
  4. Violating the Client Rights of another client or failure to uphold your Client Responsibilities at Keaton’s Place is grounds for termination from the transitional housing program. 
  5. Upon discharge the client agrees to leave the premises immediately and take all possessions immediately if found in violation of any rules noted above. 
Security Deposit

Clients are required to make a $100 deposit before moving into the Transitional Housing unless other arrangements have been made and agreed upon by Keaton’s Place staff. This deposit will be given back to the client after moving out of Keaton’s Place facilities if there are no outstanding balances.

Medical Information Release

Clients participating in Transitional Housing must sign a release of information for Keaton’s Place for all medical facilities they are associated with. This means one release of information per associated facility.

Securing Employment

Clients participating in Transitional Housing must find a job within two weeks of moving into the facility unless other arrangements have been made and agreed upon by Keaton’s Place staff.

Meeting Attendance

Clients are required to attend AND document three meetings per week. Keaton’s Place staff will provide documentation sheets.

Entering or leaving

Everyone participating in Transitional Housing is required to sign the house roster when leaving or returning to the house.

Curfew

For clients participating in Transitional Housing  you will begin in Phase I; your curfew is 9:30 p.m. for the first 30 days. Then you will transition to Phase II for 30 days, and  your curfew will be 10:30 p.m. Upon reaching Phase III, your curfew is midnight. Any violation of this will result in a Behavior Contract and reduction of privileges.

No visitors are allowed in the house overnight. All visitors must adhere to the client’s curfew and vacate the property at the time of the client’s curfew.  All visitors must remain in the common areas. No visitors may enter the house unless the person they have come to see is present. Absolutely no visitors are allowed in sleeping quarters. 

If a client leaves the Keaton’s Place housing facility due to an episode of alcohol or other drug use or a behavioral violation, the client is not permitted to visit any Keaton’s Place facilities. 

Overnight Excursions

After 30 days, if your fees are current, you can stay out overnight (limited to no more than two nights in a row)

Drug testing

Clients may be drug tested at any time by the request of Keaton’s Place staff. 

Transportation

Client is responsible for any transportation they may need. Keaton’s Place is not responsible for transporting clients that live in Transitional Housing. 

Personal Property of Guests

Belongings will be limited in amount according to space availability. We ask that you only bring a suitcase worth of belongings with you. 

Please don’t leave personal belongings in any of the common areas, i.e. bathrooms, living room. You are expected to keep your belongings in your room. WE ARE NOT RESPONSIBLE FOR THE LOSS OR THEFT OF YOUR BELONGINGS. Any theft or tampering with things that do not belong to you may result in immediate termination from the household.

The following items are not permitted in the Keaton’s Place housing: Any products containing alcohol (including, but not limited to): alcohol-based mouthwash products and alcohol-based cough syrup

Substance Use

There will be absolutely no substance use in the house. Should you use alcohol or other drugs, DO NOT attempt to enter the house. Anyone suspected of using drugs or alcohol is not permitted on Keaton’s Place property. 

The use of tobacco products and electronic nicotine delivery systems shall NOT BE PERMITTED ANYWHERE IN THE HOUSE. The use of tobacco products or electronic nicotine delivery systems is only permitted in the designated areas outside. Failure to follow this rule will result in immediate termination of transitional housing. 

Eating & Cleaning

You are responsible for providing your own food. Do not eat or tamper with anything that does not belong to you.  You will be allowed to keep food in your room, only if it is in an airtight container.

If you cook, wash your dishes and clean the eating area. There will be no eating in the bedrooms; eating is only permitted in the dining area, common area and kitchen. Please turn off all appliances when not in use, including lights, televisions, radios, etc.

Bathroom Use

Please be considerate of others with your bathroom usage. Keep the bathroom clean, including cleaning the sink and tub after each use. Do not leave towels or personal hygiene equipment in the bathroom.

Sleep in common areas

Sleeping on sofas, chairs and other fixtures in the common areas is not permitted. Everyone is provided a bed for sleeping, resting, etc. Please use it.

Maintaining Your Space

Everyone in the house is required to keep their room clean. Every resident agrees to submit to random room inspections and random drug testing by Keaton’s Place staff.

Maintaining a safe environment

Fighting and other negative behavior and/or derogatory language toward clients or staff will not be tolerated. If this happens you will be discharged immediately. You will be expected to pay for any damages that you make. ABSOLUTELY no weapons are allowed on Keaton’s Place property.

Absolutely no open flames or emissions of any kind in the home, including but not limited to the burning of incense, candles, lighters, or the use of electric or other oil diffusers, etc.  

    Medication

    The sharing of ANY medication between clients will result in immediate termination of services for all involved parties.  The misuse or abuse of over-the-counter medication is also prohibited and violation of this will result in the immediate termination from the Keaton’s Place Transitional Housing program.  

      ASked to leave

      If you are discharged from transitional housing you will be asked to leave immediately and take all possessions immediately upon departure

      If a client falls two weeks behind in client fees it may be grounds for immediate termination (Unless prior arrangements have been made and agreed upon by Keaton’s Place staff).
